SERVICES COVERED 100% TWICE PER YEAR
- Dental Examination
- Periodontal Oral Evaluation
- Fluoride Treatment (2x/year for Children up to 14)
- Teeth Cleaning (Adults or Child)
- Periodontal Maintenance (additional $50 per visit for first two visits)
- Discount on Additional Periodontal Maintenance ($135 per visit)
